(TH02) Caregivers: The Secret Sauce of Care Transitions

Caregivers have long supported post-acute and long-term care (PALTC) patients during transitions of care but are underutilized by healthcare teams, making caregivers powerful, and often neglected, allies for successful transitions. This workshop will present modern tools and models for caregiver engagement during transitions of care between PALTC, hospitals, community and micro transitions within PALTC. Breakout groups facilitated by interdisciplinary team leaders will explore resources for engaging caregivers during TOC processes (such as handoffs of medical care, medication reconciliation, and rehabilitation), ensuring equitable access for caregivers, and utilizing strengths of the interdisciplinary team. Current research on supporting caregivers of patients with dementia, COVID-19 and other conditions will be presented, providing participants with new data and ready-to-implement strategies for supporting caregivers during transitions of care in PALTC.

Learning Objectives:

At the completion of this session, learners will be able to:

List resources that empower caregivers during transitions of care from hospital to post-acute and long-term care (PALTC) settings
Describe the role of each member of the interdisciplinary team supporting caregivers during PALTC discharge processes and micro transitions
Demonstrate the ability to facilitate a successful family meeting about transitions of care through interdisciplinary collaboration and the use of evidence-based communication strategies
Demonstrate unique aspects of transitional care education for caregivers supporting patients with dementia.
