This is a presentation that will involve an occupational therapist and three physiatrists who will draw on published studies, personal experience, anecdotes, and unpublished date to educate the audience on how to optimize patient care transitions. The presenters will highlight physiatry-driven interventions, team building, family engagement, and collaboration with funding sources. Furthermore, the presenters will compare and contrast perspectives from county-based healthcare systems, academic systems, and private practice. Finally, the presenters will discuss how the ongoing COVID pandemic affects discharge planning. The presentation will consist of powerpoint presentations, case presentation, and question and answer session.
