What does Health Care Transition (HCT) look like in your state? Exploring current HCT data and innovations from Michigan

Thirty-six states and territories have prioritized NPM 12 on health care transition (HCT). To better understand HCT performance in each state and nationally, this session will explore the latest data measured by the National Survey of Children’s Health and Got Transition’s Current Assessment of HCT Activities in Care Coordination Programs. The session will cover how the data is measured, data trends over the years, and strategies to increase HCT performance. The session will also feature Michigan’s Children’s Special Health Care Services program and a discussion on their innovative HCT data collection strategies. After a group Q&A, each state will be sent their own state’s HCT data profile which can be used for Block Grant applications and reporting.
