Accreditation Services Associate, Lead Surveyor

Amy E. Hyduk-Cardillo, PharmD, MBA, BCPS has been a Lead Surveyor with ASHP Accreditation Services since 2018. Amy is a graduate of Butler University College of Pharmacy & Health Sciences and earned an MBA in Healthcare Management from Indiana Wesleyan University. Prior to working with ASHP, Amy served as System Director of Pharmacy Services for Lutheran Health Network (LHN) in Fort Wayne, Indiana. Previous to serving in the System Director role, Amy was the Clinical Pharmacy Director and PGY1 Residency Program Director for LHN. She implemented a PGY1 Pharmacy Residency Program at Lutheran Hospital and implemented and expanded multiple inpatient and ambulatory clinical pharmacy services during her 15-year tenure with LHN. Amy served on the Board of Directors for the Indiana Pharmacists Alliance (IPA), served as IPA President, and currently serves on the board for the Pharmacy Education Foundation. Amy also served on multiple hospital, local, state, and national committees and councils and served as an Indiana Delegate to the ASHP House of Delegates for several years. Additionally, Amy served as a residency program accreditation Guest Surveyor for ASHP for three years. She is a recipient of the Glen Sperandio Pharmacist of the Year Award from the Indiana Society of Health-System Pharmacists, the Pharmacy Leadership Award from IPA, and Distinguished Young Pharmacist Award. Amy now resides in Alanson, Michigan where she works remotely for ASHP.
