Transitional Care Management
I have been in Post Acute Care since 1988; starting as an Assistant Director of Nursing, the Director of Nursing, Chief Clinical Officer and Chief Learning Officer. During this time I have worked with multi facility organizations which have specialized in Assisted Living, Long Term Care, Memory Care, Short Term Care, Hospice, and Severe Mental Illness. This experience has allowed me to have a comprehensive view of post acute care.
I have served as the President of the Long Term Care Nurses Association in Illinois, am on the Board of the Illinois Health Care Association; and part of the Practice Council of AHCA.
