Jeraldine Stoltzfus is experienced in Nursing, Care Management, Quality Improvement, Risk Management and Patient Safety. Most recently Jeraldine became a Patient Safety Analyst at ECRI, where her responsibilities under the Patient Safety Organization include risk cause analysis educational presentations, deep dive event analysis and trends, process improvement and liaison services for member organizations.
She started her professional nursing career working in acute care. She transitioned to out-patient services, charged with building the Care Management Department as part of the Patient Centered Medical Home. In this role she developed a Community Clinical Resource Database, Patient Education / Care Plans and Standard Operating Procedures. This work included collaboration with the acute care center to build a robust transition of care program targeting a decreased readmission rate. Following this experience, she was a Practice Manager and Director of Quality within a Federal Qualified Health Center.
Her roles in Risk Management and Patient Safety within large Acute Care Facilities and Ambulatory Care Centers have given her the skills necessary for efficient and effective Project management, team facilitation, implementing surveys and event investigations including Root Cause Analysis. All of her experiences have engaged a culture of safety for patients and staff.
Current leadership with the ECRI and ISMP PSO includes co-chair of the Causal Functional Team. This role provides guidance, instruction, direction and leadership to the group building causal analysis programs which have been presented internationally, in addition monitors the quantitative and qualitative achievements of the team. Most recent achievement included a co-author of the white paper, Root Cause Analysis in Aging Services, Considerations for Success. Additional leadership includes co-faciliation of the Ambulatory Care Affinity Group which targets primary and speciality care and population health.
Her goal is to collaborate and build a strong sustainable approach to Healthcare safety improvement.
Jeraldine’s education includes a BA, Registered Nurse and a Master’s in Business Administration with a concentration in Healthcare Management.
