The Patient-Driven Payment Model (PDPM) created new opportunities for delivering care more and more medically complex patients. Examples include End-Stage Renal Disease and Dialysis and Ventilator Dependent Patients. During this session participants will learn how to assess your SNF market and target patient needs as well as design clinical care pathways based on new possible patients. Action steps discussed will include liaison with referral sources and payers, assessment of outside clinical support, such as access to specialists, internal clinical capabilities assessment with a focus on workforce challenges, and insights on complex clinical conditions treated in SNFs based on PDPM data.
