In this presentation, we will present evidence from a national sample of Medicare fee-for-service beneficiares that skilled nursing facilities are routinely documenting discharge goals for self-care and mobility on residents' admission assessment. However, less than half of residents have a discharge performance score that meets or exceeds their goal score. This suggests meeting or exceeding discharge goals may be a meaningful outcome for investigations of functional recovery during a skilled nursing facility stay.

Describe the Skilled Nursing Facility Quality Reporting Program quality measure for the percent of SNF residents with an admission and discharge functional assessment and a care plan that addresses function.
Summarize evidence for the relationship between documenting functional goals and functional outcomes of a skilled nursing facility stay.
