Despite increasing knowledge on the exercise recommendations and the importance of physical activity, most cancer survivors do not have access to the recommendations or any guidance on the activity advised based on their functional and medical status. During this lecture we will discuss the initiative of Sylvester Comprehensive Cancer Center and the Florida Department of Health to improve the education and access to rehabilitation services of cancer survivors based on their level of activity. This lecture will provide the audience firsthand knowledge on how to incorporate patient reported outcomes into the electronic medical record and use these results to triage survivors to the appropriate service or education needed. Furthermore, we will discuss how this experience can be extrapolated into guidelines and policies state wise.
