Molar incisor malformation (MIM) is a novel rare condition described in the literature. Patients affected with MIM usually present with normal crowns, cervical constriction, malformed, or often completely absent roots. This condition may lead to endodontic and prosthetic, as well as orthodontic treatment challenges. It may also affect the long-term survival of these teeth. This case describes the management of a 15-year-old female, with history of premature birth and controlled asthma. She presented with malformed roots and cervical constriction in maxillary central incisors, maxillary and mandibular first molars, and primary canines. Clinically, the patient presented with esthetic concerns and malocclusion. Radiographically, roots were malformed, periapical infection and bone loss in the right and left mandibular first molar area were noticed. The purpose of this report is to highlight the importance of correct diagnosis of this condition and the multidisciplinary approach needed for comprehensive management of this condition in young patients.
