Alagille syndrome is a rare genetic disorder that can affect multiple organ systems of the body including the liver, heart, skeleton, eyes and kidneys. This study describes the complete oral rehabilitation of an Alagille Syndrome patient in the operating room (OR). A 4-year-old male, with medical history Alagille Syndrome, Aorta Coarctation, Astigmatism, Myopia, born premature at 29 weeks and in rule out of Autism diagnosis at the time. The patient’s systemic health with multiple surgical interventions and hospitalization with high carbohydrate intake including prolonged bottle feeding and poor oral hygiene posed an adverse effect on his oral health.  This case presented Severe Early Childhood Caries in the initial evaluation appointment. Subsequent dental treatment and follow-up was done. Pediatric Dentist should recognize medically compromised patients because oral health problems pose increased risk to their systemic health. An improved oral health will enhance the quality of life of the patient.
