The Behavioral Health Hub offers “shared care” without demands or disruptions of your practice workflow.
Once you have identified an at-risk child through screening, an Elemy Family Navigator will facilitate the diagnostic intake process, educate parents, organize appointments, and ensure all the right information is gathered. They will also coordinate with your office and share reports and other data as appropriate.
