Impact of Prior level of functioning (PLOF) on disposition among COVID-19 patients: A Retrospective Analysis of 1314 patients

Objectives: To determine if prior level of function (PLOF) had an effect on discharge disposition and mortality among patients admitted with a diagnosis of COVID-19 and to study its association with age, ethnicity, gender and medical co-morbidities.

Design: This retrospective review looked at patients admitted between March 1 to May 31, 2020 with COVID-19, diagnosed clinically or using laboratory criteria at a 292- bed suburban community hospital.

Results: Of 1314 initial admissions, 733(56%) were male, 19.33% were African American(AA), 19.63% Hispanic(H), 50.98% White(W) and 9.97% other/unknown(O).
 
61.9% of this sample were previously independent. Mean age58.1, men(61.9%), white41%, AA:21.2%, H:25.7% with less comorbidities (DM:30%,HTN:47.8%, pulmonary:17.1%, cardiac:15.1%,mean BMI 29.9) Of these patients, 69.0% went home, 18.8% to IRF/SNF and 14.3% expired.
 
13.3%  had been previously modified independent. They were older (mean80.0),50.7%  men, mostly white 66%,AA:9.5%,H:4.7%, 38%diabetic, 78.9%hypertensive, 33.3%pulmonary disease,55.4%heart disease, mean BMI 27.9.  45.4% of these patients went home, 38% IRF/SNF, 31.7%hospice and 21.6%expired.
 
11.6% of the patients required moderate assistance. Mean age 80.0. 44.4%men, mostly white 67%,AA:16.2%,H:10.8%, 36.7%diabetic, 72.0%hypertensive, 20.0%pulmonary and  46.9% heart disease, mean BMI 26.9.  23.4% of these patients went home, 42%IRF/SNF and 36.0%expired.
 
10.9% were previously totally dependent. Mean age 78.6, 47.8 %men, white70%,AA:14%,H:6.4%, 38.4%DM, 60.2%HTN, 19.2%pulmonary, 16.2%cardiac disease, mean BMI 26.92.   23.7% were discharged home, 26.4% went to IRF/SAR and 42.3% died.

Conclusions: Higher PLOF was associated with more home discharges and lower mortality rate.  Hypertension and cardiac disease were associated with lower PLOF. There was no association with gender or ethnicity.
