P0027 - Incidence of Complications in Patients Admitted With Hypertriglyceridemia-Induced Pancreatitis in a Safety Net University Hospital

Introduction: Hypertriglyceridemia represents the third most common cause of acute pancreatitis (AP) after gallstones and alcohol. Previous studies have shown that hypertriglyceridemia-induced pancreatitis (HTG-AP) is associated with higher severity and increased rates of complications. We aimed to investigate the clinical characteristics and incidence of complications associated with HTG-AP in a unique demographic population at a community hospital.

Methods: This is a retrospective single-center study on patients admitted with acute pancreatitis between 2010 and 2019. Patients were included in the analysis based on age 18 and over, triglycerides (TG) levels above 500 mg/dl, and the diagnosis of acute pancreatitis, which required at least 2 of the following characteristics: abdominal pain, lipase elevation three times above the upper limit of normal level, and imaging features suggestive of AP. We investigated the severity of pancreatitis and the incidence of associated complications, both local and systemic, based on the revised Atlanta classification.

Results: A total of 101 (8.6%) out of 1170 patients diagnosed with AP were included in the final analysis. 84% of them were Hispanics and all of them were males. The mean age was 38 (SD, 9), median TG level was 1,630 mg/dL (IQR, 1,112-2,802), and median BMI was 30 kg/m2 (IQR, 28-34). 55% of patients had a history of alcohol abuse. Most of the patients developed mild (27%) to moderately severe (71%) pancreatitis. ICU admission was required in 45% of the patients. Acute peripancreatic fluid collection and acute kidney injury were the most common complications related to AP, found in 20% and 23% of the patients, respectively. Radiographic evidence of necrosis was present in 12 patients. Only one patient died from complications associated with AP (0.7%).

Discussion: Previous results have reported high incidence of complications related to HTG-AP. In contrast to prior studies in which most of the patients were Asians and Caucasians, our cohort comprised of predominantly Hispanic young males demonstrated a relatively stable clinical course with low incidence of complications, as well as low mortality rate. In the setting of limited demographic and a relatively small size of study population, these findings should be evaluated and confirmed in a larger population to further help characterize TG levels and complications associated with acute pancreatitis.
