P0304 - Patient Burden and Treatment Goals in the Management of Erosive Esophagitis in the United States: Results From the Study of Acid-Related Disorders (SOARD)

Introduction: The Study of Acid-Related Disorders explored US physician and patient perceptions on the disease burden of erosive esophagitis (EE).

Methods: Physicians were recruited from panels to complete an online survey recording treatment (tx) goals, satisfaction and perceived disease burden from October 2020 to May 2021. A subset completed chart review data of EE patients who received a PPI. The same patients completed a survey capturing data on symptoms, tx goals, perceptions (7-point Likert scale), satisfaction and adherence.

Results: 251 physicians (102 gastroenterologists [GIs] and 149 family/general practice physicians [FP/GPs]) completed the survey. Of these, 17 GIs and 23 FMs provided chart review data for 73 EE patients who completed the survey. Mean age was 48.2 years; 58% were women; 46% were diagnosed for < 1 year; 26% for >3 years.

Physicians and patients reported different individual symptom prevalence and levels of satisfaction (Table). Patients reported a mean of 3.2 symptoms (range 0-11); physicians reported a mean of 3.4 (range 0-11). Patients’ goals were to experience fewer symptoms (among top 3 goals for 61%), improving quality of life (58%) and fast symptom relief (41%). Only 44% of patients believed their goals were fully aligned with their physician.

A quarter of patients took a PPI more than once a day; 56% would be satisfied if they were able to take a PPI once a day. 25% of physicians were worried about the long-term safety of available treatments. 73% of physicians recommended patients add OTC therapy to their prescribed therapy; 48% of patients would feel more satisfied if they could reduce the need for supplementary OTC therapies.

84% of patients wanted long-lasting treatment options; 44% considered their current tx a long-lasting solution for their EE. Fast healing was desired by 58% and 59% of physicians and patients, respectively. 70% of patients reported that better initial healing of EE would make them hopeful that treatment would resolve their problem. Also, 51% of patients reported not being fully adherent to tx, often due to forgetting (89%); 57% reported independently increasing dose.

Discussion: Despite the availability of multiple therapies, EE patients experience a range of symptoms impacting on their ability to achieve goals. Patients reported lower levels of satisfaction than their physicians. In many, symptom control is sub-optimal, indicating a need for more effective and durable tx options.
