Introduction: Constipation-predominant irritable bowel syndrome (IBS-C) is associated with chronic bowel and abdominal symptoms which impose a significant burden to patients. Our objective was to describe abdominal symptom burden in IBS-C patients and the influence of abdominal symptoms on care-seeking behavior, healthcare resource use (HCRU), and treatment satisfaction.

Methods: A cross-sectional, on-line, general health survey collected data on US adults from Aug 2020-March 2021. Endpoints assessed descriptively for IBS-C patients (meeting Rome IV criteria) included: abdominal symptoms, medication use; office or ER visits, hospital admissions due IBS-C symptoms; and satisfaction with symptom control using a prescription (Rx) or over-the-counter (OTC) medication.

Results: Of 16,325 survey participants, 449 (2.8%) met Rome IV criteria for IBS-C. Mean age was 42.5 (SD 15.0) years, with the majority being female (76.4%) and white (79.7%). For IBS-C patients, the most reported symptom was abdominal discomfort (72.6%) followed by abdominal pain (67.3%) (Table 1); however, abdominal pain was ranked the #1 most bothersome symptom most frequently (30.1%) (Figure 1). 75.4% of IBS-C patients cited abdominal pain as the symptom that led to medication use, followed by abdominal discomfort (70.4%). Of all IBS-C patients who had seen an HCP for IBS-C symptoms in the past 12 months (61.8% [n=254]), 33.9% reported a reason for their visit was not getting adequate relief and/or worsening abdominal symptoms. Of the IBS-C patients who had an ER visit or hospitalization in the past 12 months (12.5% [n=56]), 91.1% reported abdominal pain as a symptom that led to the visit. Overall, less than half of patients currently taking an Rx or OTC reported being satisfied with the control of their abdominal symptoms. Patients were equally dissatisfied with the control of their abdominal symptoms (49.0%) and bowel-related symptoms (48.8%). Patients currently taking a Rx medication (n=103) were more satisfied with the control of their abdominal symptoms (40.8% vs. 20.7%) and bowel-related symptoms (41.7% vs. 23.9%) than patients currently taking an OTC (n=213).

Discussion: There remains a considerable disease burden and unmet need for IBS-C patients, many of whom commonly experience abdominal pain and abdominal symptoms which drive HCRU.
