P2236 - Young-Onset Colon Cancer Among People Living With HIV

Introduction: Colon cancer is a leading cause of cancer-related mortality in the United States (US). For persons at average-risk of developing colon cancer, guidelines have lowered the age to start screening to 45 years. There is limited data on lower gastrointestinal cancers for People Living With HIV (PLWH). Atlanta currently has the highest rate of HIV diagnosis in the US. We evaluated the clinical presentation of colon cancer in those with and without HIV at a hospital in Atlanta.

Methods: Data was obtained from the electronic medical record system. We included patients who were diagnosed with colon cancer (CC) between January 01, 2015 and December 31, 2019, using ICD-9 and ICD-10 codes. Patient were stratified by HIV status. Descriptive statistical summary was performed for gender, age, race/ethnicity, insurance status, smoking/drinking status and BMI. Means with standard deviations were used for numerical variables and frequencies with percentages were used for categorical variables. Two sample t-test was performed to test the numerical variable differences between CC only and HIV and CC, and Chi-Square test was performed to test the categorical variable differences between CC only and HIV and CC. SAS 9.4 was used for all the data analysis and p< 0.05 was considered statistically significant.

Results: Of the 1,482 patients, 114 had HIV. Among CC Only, the number of males and females were similar, whereas among HIV and CC, there were more males than females (Table 1). The average age of the CC only patient sample was 61.62 and the average age of the HIV and CC patient sample was 51.31. Males were diagnosed with CC within 3.31 years of HIV diagnosis and females were diagnosed with CC within 2.52 years of HIV diagnosis. Persons with HIV and CC were more likely to have a lower BMI and more likely to have history of smoking and alcohol use, compared to CC only.

Discussion: Our study demonstrated that patients with HIV were more likely to be diagnosed with young-onset colon cancer compared to the general population. They were also likely to be diagnosed with colon cancer within 5 years of HIV diagnosis. Larger observational studies should be done to assess whether HIV is an independent risk factor for colon cancer and whether this population should be screened at a younger age.
