Introduction: Left-sided cholecystosis is the presence of the gallbladder on the inferior aspect of the liver to the left of the hepatic condyles or sickle interstitium, without visceral retroversion. The frequency is estimated to be 0.3-0.2%. Although hepatic developmental abnormalities are rare, hepatic tissue may occur in areas that are not in contact with the liver parenchyma, which is called ectopic liver. It is often detected during surgery or autopsy, but the frequency is only about 0.09% to 0.47%. In this article, we report a case of ectopic liver combined with left-sided gallbladder, which was discovered by chance during a thorough examination of another disease, with a discussion of the literature.
Case Description/Methods: The patient was a 49-year-old man. During a contrast-enhanced CT scan, a nodule-like structure was found between the liver and gallbladder, and the patient was referred to surgery. Laparoscopic surgery was started at 4port, but one port was added to the left upper quadrant because it was found to be a left-sided gallbladder with a right-sided hepatic conduit. Resection of the node-like structure between the liver and gallbladder and cholecystectomy were performed. Histopathological diagnosis of ectopic liver was made. The patient was discharged from the hospital on the fourth postoperative day without any postoperative complications.
Discussion: Ectopic liver is difficult to diagnose preoperatively because of the immaturity of the vascular supply and bile excretion mechanisms and the peculiarity of the site that there is no pressure drainage by surrounding tissues. It is difficult to resect the left side of the gallbladder by the usual laparoscopic cholecystectomy method, and it can be safely performed by placing the port in the left upper quadrant.
