The interprofessional team conference is a key forum to facilitate communication for inpatient rehabilitation teams, yet optimal communication dynamics are challenging to define and execute. This symposium will provide an overview of the implementation interventions utilized to change the communication culture of team members with a focus on shared outcomes interpretation and data-driven team decision-making. We will review a novel metric, the Ability QuotientTM , and its associated data dashboard, which facilitated the development of a shared team focus on patient progress and predicted performance. This presentation will address additional implementation interventions to achieve translation of the Ability QuotientTM into team conferences and enhance interprofessional collaboration.
