This symposium will outline the application of evidence-informed knowledge translation practices within the healthcare context.  After defining knowledge translation and providing an overview of key ingredients in dissemination, implementation, and evaluation in healthcare (Brownson et al., 2018), we will share applications within the Military Health System, describing both successes and challenges.  We hope to empower attendees with the knowledge, tools, and resources to tackle knowledge translation in their settings.  By incorporating both the perspective of a clinician stakeholder as well as knowledge translation implementor, the discussion and recommendations will be balanced and centered around functional, practical behavior change grounded in a solid theoretical framework.

• Identify 2 barriers and 2 facilitators of knowledge translation in your own setting.
• Select 2 specific knowledge translation tools, templates, or resources and develop a plan to utilize in your own setting.
