This session will provide an overview of the incidence of preventable adverse events and medical errors. Faculty will describe a tool to help prepare for disclosing errors and providing apologies. Simulated conversations will demonstrate suboptimal and optimal ways to share information. Attendees will have an opportunity to practice disclosing errors through role play, with feedback provided.
