This session will summarize the understanding of and need for coordination of lifelong care for individuals with congenital heart defects. Attendees will explore the roles of the primary care provider in concert with the specialty cardiology provider in implementing clinical guidelines and best practices related to lifelong care and transition. Resources will be shared to facilitate working with patients and families in the medical home and transition to self-care and adult-oriented systems as appropriate.
